If the Vanos hasn't been touched in 260K, I would do it preemptively. M52TU was the worst in symptoms for bad Vanos, followed by M52, with M54 being the mildest symptoms. But I bet it's bad, even if it's not that bad. For an afternoon and a hundred bucks, do it.
